Healing to Wellness Court Case Manager

SALARY RANGE: $47,900.00-$71,900.00

SUMMARY:
The Healing to Wellness Court (“HTWC”) Case Manager has primary responsibility for assessing individuals for admission to the HTWC, monitoring compliance with program requirements, providing participant case management services, documenting participant progress, and information sharing with the Court, the HTWC Team, and other partnering jurisdictions. The HTWC is a specialized docket of the Mille Lacs Band Court of Central Jurisdiction which uses a multidisciplinary collaborative team approach to assist court-involved individuals seeking to recover from addiction to alcohol and/or other substances.

QUALIFICATIONS:
• Bachelor's degree (B.A. or B.S.) in Counseling, Human Services, Social Work, Psychology, Criminal Justice or other relevant field required.
• Five years working with direct contact with participants in a substance abuse treatment setting required. Mental health background a plus.
• Comparable education and experience will be considered. Additional qualifying experience may be substituted for the education requirement on a year-for-year basis.
• Knowledge of concepts of drug courts/Tribal Healing to Wellness Courts, addiction, substance use disorder, chemical dependency treatment, and addiction recovery required.
• Familiarity with Trauma Informed and Motivational Interviewing (MI) techniques desirable.
• Knowledge of child abuse and neglect court case processes and the federal Indian Child Welfare Act, 25 U.S.C. § 1901 et seq., desirable.
• Cultural competence, or willingness to develop competence, in the Mille Lacs Band of Ojibwe’s culture, customs, and traditions.
• Superior writing skills and excellent communication skills required.
• Excellent organization and interpersonal skills required.
• Experience in leading and participating on a team in a professional setting required.
• Strong personal computer skills in a Windows and Microsoft Office environment, especially Excel spreadsheet development, required.
• Must be willing to travel to the necessary court trainings & seminars as required.
• Must have a valid current driver license.
• Must have a valid driver’s license
• Must be insurable under the Mille Lacs Band Drivers Insurance policy.
• Must pass a background check with no felony convictions within the past 10 years.
• Must pass a pre-employment drug & alcohol test.

DUTIES AND RESPONSIBILITIES
Working with the courts and collaborating with an array of treatment, social services, housing, employment and community-based organizations to provide treatment services to clients as indicated below.

Provides limited work direction to the HTWC Team, and Court Administrative Staff regarding program and program outcomes.

Screening and Assessment
• Provides program information to applicants and other interested persons.
• Conducts screening and assessment of HTWC applicants and reports outcome to HTWC Team.
• Refers program applicants for substance abuse evaluations.
• Assists participants to complete necessary HIPAA-compliant consent to release information forms.
• Makes recommendations to the HTWC Team regarding program admission.
• Serves on the HTWC Team.

Case Management and Monitoring
• Conducts new participant orientations
• Provides intensive supervision and case management for the HTWC program.
• Develop participant’s plan based on the information provided during the assessments and facilitate appropriate level of care referrals, and revise as needed, as discussed in HTWC program team meetings.
• Monitors participants’ compliance with the plan and program requirements as established by the HTWC program.
• Conduct, or cause to have conducted, observed drug and alcohol testing.
• Provides group facilitation and crisis intervention as needed.
• Coordinate inter-agency cooperation and collaboration for pertinent services including treatment, social services, housing, education, employment, vocational training, medical, mental health, and legal to ensure that each participant meets all criteria for phase movement and graduation.
• Assists in administering the case management database.
• Maintains participant files and prepares statistical reports on participant progress.
• Complete HTWC Participant Progress Reports and distribute to HTWC team in advance of case staffing and status hearings.
• Devise and propose participant incentives to reward success and consequences to hold participant accountable for program non-compliance for HTWC Team approval.
• Coordinates pro-social and cultural activities for participants and their family.
• Plan and organize participant graduation ceremonies.

Development and Advocacy
• In concert with the HTWC Team, organize periodic HTWC Team and participant events, such group family dinners or sober holiday celebrations.
• Attend relevant trainings and conferences.
• Communicates with HTWC Team members, local drug courts, and Band and community resources as needed.
• Engages in outreach efforts to the Mille Lacs Band communities.
• Represents and promotes the program concepts to outside entities.
• Researches new emerging and best practices in drug and alcohol testing methods and evidence-based treatment modalities and shares with the HTWC team.
• Assists in ensuring processes and operations used in the HTWC comply with established program goals.
• Identifies grant and other funding opportunities to sustain the HTWC and participates in grant application development.
• Performs additional various diversified tasks and assignments as may be required in connection with the successful operation of the HTWC.

WORKING CONDITIONS:
• Nature of work is such that incumbent experiences regular periods of moderate to high stress levels. Primary sources of stress are in dealing with participants and families’ needs and issues, the workload, and meeting timelines.
• Work is almost exclusively indoors in a controlled climate area.
• Little threat of personal danger or risk.
• Hours are typically 8-5, but extra hours may be necessary to meet participants’ needs. May involve some evening or weekend working.
• Local travel is required, limited travel outside the state is also required.
