Waivered Services Support Aide Part Time DIII

SUMMARY:
The Waivered Services Support Staff develops and coordinates community elder programs for disabled adults and Elders. Provides community outreach to all districts.

QUALIFICATIONS:
• Associate’s degree is required;
OR
• High school diploma or GED with at least three (3) years prior experience working with elders/disabled adults is required.
• Previous experience with elders/disabled in a public or private social service agency is preferred.
• Ability to manage time effectively.
• Ability to communicate effectively in writing or orally.
• Knowledge of Data Privacy regulations and ability to keep confidential information.
• Knowledge and experience working with Native American communities, culture, customs, and cultural law.
• Must have a current valid Minnesota driver’s license, reliable personal transportation, and proof of current insurance coverage.
• Must be insurable under the Mille Lacs Band Drivers Insurance policy.
• Must pass a background check.
• Must pass a pre-employment drug and alcohol test.

DUTIES AND RESPONSIBILITIES
• Provide services in the homes of elders/disabled who need advocacy services.
• Participate in case plan development and implementation to carry out specific goal achievement of case plan.
• Identify and work with elders/disabled needs, challenges and strengths.
• Communicate and work closely with the Waiver Services Relations Liaison Manager assigned to the case by relaying critical information and coordinating duties.
• Assist elders/disabled from various activities and healthcare related travel.
• Provide transportation as needed.
• Provide light housekeeping as needed.
• Refer clients to the Waivered Services Benefits Coordinator as needed.
• Assess the need for and obtain resources for the elders/disabled.
• Attend review meetings and other community meetings, as needed.
• Maintain files in conjunction with the Waiver Services Relations Liaison Manager.
• Contact with clients in the community and the office.
• Maintain case/progress notes and records/logs.
• Companionship; provide socialization, create a trusting relationship, play cards for example.
• Other duties as assigned.

WORKING CONDITIONS:
• Hours vary and are based on client’s needs. Occasional work on holidays and weekends.
• Regular periods of moderate to high stress levels. Primary sources of stress deals with clients’ issues and workload.
• Moderate threat of personal danger or risk.
• Local travel is required; limited travel outside the state is also required.
• Work from home may be permissible with a signed telework agreement.
