SALARY RANGE: $34.00 - $61.53

SUMMARY:
The Population Health Manager is responsible for the oversight and supervision of the Mille Lacs Band of Ojibwe’s Population Health Program. This person will manage the various projects, grants and services available to the community members through Population Health Programs.
QUALIFICATIONS:
• Bachelor’s degree in nursing, public health., social work or health related field is required. Master’s Degree in area of study preferred.
• Three to five years’ experience in a public health setting
• Three to five years’ experience in management and leadership
• Knowledge of an understanding of health promotion and disease prevention and the ability to teach others these principles
• Knowledge of tribal, county, state and federal health and human services required
• Ability to work collaboratively across HHS entities and disciplines.
• Excellent communication and interpersonal skills to include the ability to negotiate
• and resolve conflicts and build teams.
• Demonstrated ability to create win/win solutions and relationships
• Demonstrated strong customer service skills to meet internal and external client/patient expectations.
• Demonstrated organizational, planning and project management abilities.
• Demonstrated understanding of quality improvement processes
• Ability to function independently and deal with multiple, simultaneous projects.
• Ability to implement change in a positive, sensitive and forward- thinking manner
• Demonstrated ability to effectively manage grants
• Demonstrated understanding and compliance with HIPAA
• Experience working within the Native American Culture
• Must have current valid driver’s license.
• Must be insurable under the Mille Lacs Band Drivers Insurance policy.
• Must pass a pre-employment drug and alcohol test.
• Background investigation is required
DUTIES AND RESPONSIBILITIES
• Supervision of assigned staff
• Assures that programs are properly resourced to allow for success
• Manages and directs all activities within area of responsibility.
• Continually assesses all services, identifies problems, utilizes data to analyze and propose innovative approaches for solutions.
• Maintains records related to operations and services that are complete, accurate, available, and in compliance with all legal, regulatory, and policy requirements.
• Holds self and others accountable to policy, standards and commitments and provides timely follow through on questions and concerns.
• Ensures development of Population Health initiatives to improve patient satisfaction and family centered care.
• Manages assigned grants to ensure grant requirements are met
• Educate staff, peers, and supervisors regarding current community health issues
• Manage an effective public health process based on regular assessment of the community, regular evaluations of program outcomes and updating of planned activities
• Participation and collaboration with other HHS Departments/Programs to deliver services in an integrated whole-person model
• Assure compliance with grant programing, Band and Federal employment policy, regulatory standards of care, policies and procedures.
• Develops and operationally manages programs and initiatives with measurable outcomes.
• Formulates objectives, goals and strategies collaboratively with other stakeholders.
• Provides reports to leadership team outlining programs and progress toward meeting goals and objectives.
WORKING CONDITIONS:
• Hours are typically 8-5, but extra hours may be necessary to meet deadlines and maintain workload.
• Stress level is moderate however can escalate at certain times
