SALARY RANGE: $49,000-$72,200

SUMMARY:
The Reentry Case Manager is responsible for case management for the reentry and integration of offenders released from a correctional facility. They work with participants and in partnership with team members to help prepare individuals to transition into the community. This role involves developing reentry plans, providing resources and support and coordinating with community organizations to ensure a smooth transition.
QUALIFICATIONS:
• Associate’s degree in vocational counseling, social work or a related field and a minimum of three (3) years of experience working directly in the human services field or; Five (5) years of case management experience is required.
• Three (3) or more years of experience in working with the Native American community, including a knowledge of and respect for Native American culture is required.
• Knowledge of interviewing and information gathering techniques is required, one (1) year of interviewing/assessment experience is preferred.
• Proficient in Microsoft Office systems.
• Must have experience with data entry and working with database systems, two (2) years’ experience is preferred.
• Ability to maintain a professional relationship with clients is essential to be able to make fair, unbiased decisions based exclusively on eligibility.
• Skills in job counseling, employability assessments, and formulation of client improvement or development plans are preferred.
• Experience monitoring and documenting participants, knowledge of conflict resolution and motivational techniques, and problem identification and solving is preferred.
• Ability to present information effectively to individuals and groups.
• Provide information and referral to community resources to help participants address their employment and life management barriers that could affect ability to achieve career goal.
• Assist with the facilitation of orientations, individual or group.
• Enter all data in electronic case management system with 48 hours of service delivery –
• Ability to deal with highly confidential information and comply with data privacy laws.
• Ability to establish and maintain effective working relationships with participants, public, co-workers and outside agencies.
• Work closely with other team members, communicating regularly, attending training and team meetings, etc.
• Strong organizational, communication and interpersonal skills.
• Must have current valid driver’s license, reliable personal transportation and proof of current insurance coverage.
• Must be insurable under the Mille Lacs Band Drivers Insurance policy.
DUTIES AND RESPONSIBILITIES:
• Provide individual case management sessions with all participants at regularly scheduled intervals.
• Conduct initial screenings to determine the level of service and support needed.
• Conduct in-depth assessments of participants’ needs, strengths, and challenges, along with offering pre- and post-release support through intake, interviews, assessments, and orientation sessions.
• Develop individual appropriate case plans that identify barriers to successful reentry.
• Provide regular updates to the Program Manager and team members.
• Work closely with internal and external agencies to provide tailored support addressing the individual’s needs, including assistance with sobriety, community reintegration, cultural-based programs, and accessing essential services.
• Refer participant to other programs as appropriate.
• Develop a mutually agreed upon development plan for all participants and coordinate and monitor these plans to ensure all necessary services are available and accessed.
• Ensure that the plans are in compliance maintaining adequate documentation of all cases and coordinate with other agencies and/or social service organizations that have a role in the plan.
• Coordinate and monitor services that may be needed by the participant to obtain employment. Such services may include child care, transportation, job counseling and social services. Refer participant to counseling, peer group support or other services as appropriate. Update plan as appropriate.
• Maintain files and documentation on each case to ensure compliance with state and federal requirements and to ensure due process. Maintain records necessary to support compliance or non-compliance assertions. Maintain informational releases as required by the Data Privacy Act.
• Perform case maintenance in the appropriate database or registry system.
• Assist in the collection and compilation of demographic and statistical information to evaluate the effectiveness of programs and identify gaps in services.
• Represent the Department to participants, officials, and other reservations and communities.
• Conduct public presentations of the programs as required.
• Other duties as assigned.
WORKING CONDITIONS:
• Work is almost exclusively indoors in a controlled climate area.
• Moderate stress level due to meeting program goals and working with participants.
• Little threat of personal danger or risk.
• Hours may vary depending upon departmental needs and may include evenings and weekends.
• Remote work capabilities with supervisor approval. Must follow Remote Work protocol.
• Travel in-state is required.
