Men's Half Way House Renovation

THIS IS NOT A CHANGE ORDER NOR A DIRECTION TO PROCEED WITH THE WORK DESCRIBED HEREIN.

The Mille Lacs Band of Ojibwe, Health and Human Services is accepting bids for the Men’s Half Way House Renovation, located in Onamia, MN. Bids will be due Wednesday 8 June, 2022 at 3:00 PM. Bids received will be opened and qualified by the Mille Lacs Band of Ojibwe on Thursday 9 June 2022 at 10:30 AM.

General Notes:

  1. It is the Contractors responsibility to identify any and all discrepancies in the scope of work, not meeting Industry Standards or that which is inconsistent with the International Building Code (IBC).
  2. All electrical wiring, apparatus and equipment for electric light, heat and power, technology circuits or systems shall comply with the rules of the Department of Commerce or the Department of Labor and Industry, as applicable, and be installed in conformity with accepted standards of construction for safety to life and property.
  3. Contractor must reconnect all utilities, service panel or service feed. Also includes gas, propane. Include such equipment or materials identified in the pre-bid conference.
  4. Contractor will secure all permits and fees.
  5. Contractor is responsible for a thorough investigation of the scope of work.
  6. Contractor will repair any damage to the property or structure created by the scope of work.
  7. Contractor shall be responsible for all debris removal related to all work performed under this work scope.
  8. Contractor to carry Builder’s Risk insurance on project for duration of construction.
  9. NO WORK SHALL BE PERFORMED UNTIL ALL REQUIRED PERMITS HAVE BEEN ISSUED AND COPIES IN THE POSSESSION OF THE PROJECT COORDINATOR AND /OR MILLE LACS BAND BUILDING OFFICIAL.

HEALTH AND HUMAN SERVICES WILL, TO THE GREATEST EXTENT FEASIBLE, GIVE PREFERENCE IN THE AWARD OF CONTRACT TO INDIAN ORGANIZATIONS AND INDIAN-OWNED ECONOMIC ENTERPRISES.

Work Scope:

Project Narrative:  The Health and Human Service’s department identified some needs for upgrades and renovations for the Men’s Halfway House. This is a Department of Justice Grant Funded Project and the scope of work will be included below.   


 General Structure:

a. Provide temporary barricades and protection as necessary for all work being completed.
b. Provide labor and material required to remove and dispose of the existing flooring and carpet bonding agents throughout the entire house, stripping down to sub-flooring for new sheet vinyl. Prep sub-flooring for installation of new sheet vinyl.
c. Provide labor and material required to remove and dispose of the existing sheet vinyl throughout entire kitchen and both bathrooms. Prep sub-flooring for new sheet vinyl.
d. Provide labor and material required for removal of existing toilet and pedestal sink in bathrooms as necessary for vinyl flooring removal and replacement.
e. Provide labor and material required to furnish and install 88 lineal feet of 5" seamless K style gutter at least .027 gauge on the North and South sides of the existing building (44' per side).
f. New gutter to include (2) 15' downspouts, with concrete splash blocks.
g. Provide labor and material required to furnish and install new vinyl flooring throughout entire house. Includes lower level as well. Vinyl flooring at least 4mm thick with pattern.
h. Provide labor and material required to furnish and install oak shoe around perimeter of the living room, kitchen, bathrooms and all bedrooms, after installation of new vinyl flooring.

 First Level:

i. Provide labor and material required to remove three (3) existing fluorescent light fixtures in the kitchen and corridor. Replace with LED light fixtures.
j. Provide labor and material required to remove two (2) existing fluorescent light fixtures in the living room. Replace them with ceiling fans with LED light kits.
k. Provide labor and material required to furnish and install one (1) Kitchen Range Hood with integral fan. Include all ductwork to exterior and roof penetration. Flash and seal any roof penetrations required.
l. Provide labor and material required to re-paint the ceiling in the kitchen, living room and corridor.
m. Provide labor and material required to remove and dispose of the existing fiberglass tub / shower in the first-floor bathroom.
n. Provide labor and material required to demo and then re-frame the existing partition walls in the bathroom as necessary to accommodate new handicap walk in bathtub.
o. Provide labor and material required to provide waste, water, and vent piping for new handicap tub.
p. Provide labor and material required to furnish and install new gypsum drywall waterproof green moisture resistant board and vinyl wainscoting around perimeter of new tub. Including all finish work – mud, taped, sanded, and painted.
q. Provide labor and material required to patch all existing drywall partition walls in the first-floor bathroom.
r. Provide labor and material required to prime and paint first floor bathroom. Match building standard.
s. Provide labor and material required to reseat original toilet and bathroom sink. Includes new faucet and drain for handicap tub.

 Lower Level:

t. Provide labor and material required to remove and dispose of eleven (11) lower-level bedroom windows.
u. Provide labor and material required to cut open existing wall as necessary to enlarge the exiting rough opening as necessary to provide egress openings for elven (11) window locations.
v. Provide labor and material required to re-frame existing window opening as necessary to enlarge for the new egress windows (11) locations.
w. Provide labor and material required to patch the existing gypsum drywall around the (11) enlarged window openings Including all finish work – mud, taped, sanded and painted. Match building standard.
x. Provide labor and material required to furnish and install new ranch casing around the perimeter of the new eleven (11) egress windows. Match material of existing windows.
y. Provide labor and material required to patch the existing vinyl siding around the eleven (11) enlarged window openings. Match building standard as close as possible.
z. Provide labor and material required to furnish and install eleven (11) egress casement windows. Includes all egress window hardware.
aa. Provide labor and material required to prime and paint all lower-level bedrooms that received a new egress window.
bb. Provide labor and material required to remove and dispose of the existing fiberglass tub / shower in the lower-level bathroom.
cc. Provide labor and material required to demo and then re-frame the existing partition walls in the bathroom as necessary for new walk-in shower.
dd. Provide labor and material required to furnish and install new 60" x 30" fiberglass shower stall and enclosure.
ee. Provide labor and material required to reset original toilet and bathroom sink. Includes new faucet and drain for walk-in shower.

  1. Bids must be honored for 90 calendar days.

  2. Anticipated project schedule is Summer/Fall for construction.

  3. Contractor shall include 0.5% TERO tax fee in base bid. Contractor shall utilize FEDERAL Davis Bacon wage rates for construction of this project. Wage determination rates for Mille Lacs County will be provided in the first pre-bid addendum.

  4. Project is sales tax exempt. Awarded contractor will be provided with the tax exemption form.

  5. The Men’s Half Way House facility is a service focused facility that operates 24 hours, seven days a week. Plans will need to be made so there is as little disruption to the program as possible. Continuous cleaning operations, temporary barricades and other measures will need to be taken to limit disruption to daily building operations. Building access points will be discussed at the pre-bid site visit.

  6. Contractor shall be required to provide portable toilet facilities for work staff.

Specified Product Substitutions: All proposed product substitutions shall be submitted in accordance with the project specifications. Any approved substitutions, will be made available to all bidders via Pre-Bid Addendum.

Mobilization:

  1. The Contractor shall be capable of mobilizing his equipment and crews within seven days of the receipt of Notice to Proceed.
  2. Contractor shall submit building schedule to Owner at the time of contract signature by the Contractor.
  3. Contractor shall provide means and methods for all building phases of construction.

Bidding notes:

  1. Submit signed proposal in lump sum (supply and install), not to exceed amount
  2. All Contractors (including subcontractors) must comply with Commercial Davis Bacon wage requirements (Mille Lacs County).
  3. All Contractors must provide the following along with their bid submittal:
    a. Completed and signed MLB Community Development Construction Bid Form
    b. A copy of Current MLB Vendor’s License (or a copy of the submitted application)
    c. A copy of Current Insurance Certificate
    d. A copy of Subcontractor/Material Supplier list
    e. A copy of Authorized Signature Sheet (submitted with first bid submittal)
  4. All Contractors must comply with all Mille Lacs Band of Ojibwe American Indian Employment requirements (see 18 MLBSA § 5). Contact Lori Trail at (320) 532-4778.

**Please note that the bids must be submitted via mail to the P.O. Box.
FedEx and UPS will not deliver to a P.O. Box and the Onamia post office will not accept hand delivered items. Please plan accordingly to ensure the timely receipt of your bid submittal.

**The Band reserves the right to reject any bid that it is unable to collect at the Onamia post office by the bid deadline date and time, provided that the Band has made diligent and reasonable efforts to collect the bid. The Band reserves this right even in the event that the bid has been postmarked before the deadline.

PROPOSALS NOT SUBMITTED IN THIS MANNER WILL BE REJECTED.

HEALTH AND HUMAN SERVICES RESERVES THE RIGHT TO REJECT ANY AND ALL BIDS FOR ANY REASON.

PERMIT AND CONTRACTOR REQUIREMENTS:
Permits: Contractors are responsible to attain all necessary permits for all work, including Mille Lacs Band of Ojibwe (MLBO) Permits.

Licensing:

  1. Firms must be licensed with the Mille Lacs Band of Ojibwe. A copy of this license (or the license application) must accompany each bid. Licensing process can take several weeks. If you are not currently licensed with the MLBO, please submit a copy of your license application along with your proposal. Bonding Requirements:

Performance and Payment Bond: A performance and payment bond on the part of the contractor for 100 percent of the contract price shall be required. “Performance and Payment Bond” means a bond executed in connection with a contract to secure fulfillment of all the contractor’s obligations under such contract.

Bid Bond: Provide a bid guarantee equivalent to 5 percent of the bid price. The bid guarantee must consist of a firm commitment such as bid bond, certified check, or negotiable instrument accompanying a bid as assurance that the bidder will, upon acceptance of his/her bid, execute such contractual documents as may be required within the time specified after the forms are presented to him/her.
